Family Care Partner Information and Skills

Memory Care Home Solutions will make it easier for you to enhance your skills and adapt your home setting to meet the ever-changing demands of caregiving. Over the course of one year, Memory Care Home Solutions will provide you with in-home, personalized care partner education to:

  • Reduce your stress
  • Improve communication with the person living with memory loss
  • Make your home a safer environment for you and your loved one with dementia
  • Modify daily activities to promote independence for your loved one with memory loss
  • Learn ways to reduce difficult behavioral symptoms

In addition, we personally connect you to community resources and other supports to specifically meet your needs. Follow-up consultations will be scheduled to ensure your continued success.

Dementia Navigation

All new families enter MCHS through dementia navigation. Each person living with dementia and their family are paired with a dementia navigator and clinical team to walk the journey of dementia together. Following a comprehensive assessment, personalized care plans are created based on your family’s priorities and goals. Through monthly visits, navigators offer education, connection to community resources and strategies to enhance well-being and reduce stress.

Occupational Therapy

Occupational Therapy (OT) begins with an assessment of the person living with dementia, the home environment, and the care partner. Caregivers receive a detailed report with evaluation findings and learn skills and strategies to support the person living with dementia. OTs work with clients through in-home or telehealth appointments to improve activities of daily living, address behavioral symptoms, and explore assistive technology and adaptive a equipment to support daily function and home safety.

Individual Counseling

MCHS offers short-term, solutions-focused counseling for dementia care partners. Clients meet with a mental health professional to discuss their current challenges and build strategies specific to their health and well-being as a care partner. Counseling is ideal for care partners who want support in areas such as processing grief and loss, identifying strengths and boundaries, and practicing communication skills for positive relationships.
