While the GUIDE model is only available as a Medicare benefit to people enrolled in Medicare Parts A and B, we offer other supportive services to people living with dementia and care partners, regardless of insurance coverage. What is the GUIDE model?

The GUIDE (Guiding an Improved Dementia Experience) Model is a new Medicare program that offers enhanced care for people living with dementia and family/friend care partners, including the personalized support of a dementia navigator and a team of healthcare professionals, to help you manage dementia progression at home.

Am I eligible?

Basic Requirements:

  • Must be enrolled in Original Medicare Parts A and B
  • Patients are free to see any doctor or hospital that accepts Medicare

How to Access GUIDE Services:

  1. Learn about GUIDE or be identified as a potentially eligible patient
  2. Visit with a doctor or care team participating in GUIDE for a comprehensive assessment
  3. Your doctor or care team will submit your eligibility form to CMS
  4. If eligible, you’ll receive information about next steps from the care team

Experience and Expertise

Memory Care Home Solutions is the only organization in the nation that has been offering dementia navigation services since 2002! The interdisciplinary staff of Dementia Navigators, Occupational Therapists and Social Workers have been walking the journey of dementia progression alongside thousands of families for more than two decades. Before there was a Medicare benefit for comprehensive dementia care and support, MCHS was advocating for this standard of care and raising grants and philanthropic funds to provide services to every family who could benefit. Our results and lessons learned from two decades of delivering dementia navigation informed the Centers for Medicare and Medicaid Services as they were designing this model.

Dr. David Carr, our medical director, is a professor of medicine and neurology at Washington University and highly renowned for his clinical and research contributions in geriatric care, rehabilitation and dementia.

People living with dementia who participate in our programs spend more time at home, less time at the hospital, and enjoy an improved quality of life. Care partners report less stress, more confidence to manage care, and improved well-being. Memory Care Home Solutions works with external evaluators to be sure our programs are effective for the families we serve. We publish our results in peer-reviewed journals and present our work and outcomes at professional conferences across the country.

MCHS is closely connected to the village of community partners, resources and service providers who help older adults, people living with dementia, and family caregivers thrive. When we make a referral to a community organization, we are connecting families with trusted partners with whom we have a relationship.

 

Radical Compassion

MCHS was founded in 2002 by a family care partner, Lisa Baron, based on her family’s challenges supporting their beloved Betty Baron through her Alzheimer’s diagnosis. We are governed by a Board of Directors comprised of family caregivers and staffed by former and current dementia caregivers. We meet every family with acceptance, understanding, joy, and support – exactly where they are in their path to understanding and living with dementia. We acknowledge that people affected by dementia and their care partners are the TRUE experts on this condition.
