The Dementia-Capable Care Network is an interorganizational, community-based, collaborative model. Through a person-centered approach, the network aims to fill gaps in dementia service systems for persons with Alzheimer’s disease and related dementias (ADRD) and their care partners. The network brings together academic partners, national advocacy and support organizations, community-based organizations, hospitals, organizations that empower individuals with developmental disabilities, and quasi-government organizations regularly to discuss current systems, needs, and opportunities within the community. The Dementia Care Network will serve as an advisory and network building committee dedicated to improving dementia care services in the region. Current partners include:
- Alzheimer’s Association of Greater Missouri Chapter
- Association on Aging with Developmental Disabilities
- Memory Care Home Solutions
- Memory Diagnostic Center and Alzheimer’s Disease Research Center
- Perry County Memorial Hospital
- St. John’s Community Care
- St. Louis ARC
- St. Louis Area Agency on Aging
- St. Louis University Gateway Geriatric Education Center
- Washington University Evaluation Center
